Within Reach is a hospice care organizer built for real families and primary caregivers. Keep notes, medications, appointments, and daily tasks in one place—so nothing important gets lost on the hardest days.

Whether your loved one is at home or in a facility, caregiving can quickly turn into juggling sticky notes, group texts, and half-remembered conversations with the nurse. Within Reach gives you a clear, shared picture of what’s happening, so you can stay organized and show up prepared.

Designed for real hospice life

Capture what you see and feel, in the moment
Log symptoms, changes, and important details using structured notes with date, time, and tags (like pain, breathing, mood, sleep, nutrition, and more). Use it as your “brain” when the nurse or doctor comes back around.

Keep every visit and appointment straight
Track doctor visits, nurse visits, social worker check-ins, chaplain visits, and non-medical errands all in one schedule. Add locations or telehealth links and see what’s coming up at a glance.

Stay on top of medications
Build a medication list that actually makes sense: what each med is for, how often it’s taken, and any notes you need to remember. Check things off as they’re given so everyone knows what happened and when.

Share responsibilities without losing control
Set up roles like Patient, Primary Caregiver, and Family Support so the right people can add notes, update tasks, or just stay informed—without giving everyone full control over everything.

One secure space per family
Use a unique family code so only your people can see your loved one’s information. Each family’s data stays separated and private, no matter how many caregivers use the app.

Within Reach was designed for the realities of end-of-life care: constantly changing symptoms, rotating visitors, and hard decisions. It’s meant to reduce the chaos—not replace your hospice team.

Nouveautés (v3.0)

With this release, WithinReach introduces one of our biggest updates yet, with major improvements across the app to make caregiving tasks feel clearer, more organized, and easier to manage day to day. The biggest visual changes are in the Care Log and Schedule experience, where we’ve refreshed the design and improved the overall flow so important information is easier to find, easier to update, and easier to act on. These updates were built to help families and caregivers spend less time navigating the app and more time focusing on care.

The Care Log has been redesigned with a cleaner, more modern experience that makes it easier to review updates, capture information, and stay aligned with the rest of the care team. We’ve improved the structure and presentation so entries feel more readable and the screen is easier to use across different device sizes. The result is a more polished space for tracking care-related updates and keeping everyone on the same page.

We also made major updates throughout the Schedule section. Appointments, tasks, medications, and tracking-related screens have all been improved to create a more consistent and intuitive experience. Across these areas, we focused on making details easier to review, actions easier to take, and important care information easier to understand at a glance. Whether you are checking what is due today, reviewing an appointment, logging medication activity, or updating a care-related task, the experience should now feel more streamlined and easier to navigate.

Medication and care tracking workflows also received meaningful improvements in this release. We refined the way medication activity is displayed and logged so day-to-day tracking is clearer and more reliable. Time and date information is shown more naturally in several parts of the app, helping screens feel easier to scan and reducing confusion when reviewing reminders, schedule items, or recent activity. We also made a number of smaller interface refinements across key screens so layouts feel cleaner, touch targets are easier to use, and information is presented more consistently.

Sign-in and account access have also been improved. This release adds support for simpler sign-in options, including Google and Apple, while also strengthening account protection with additional verification steps. Depending on your account setup, you may now be asked to verify your email address or phone number, and some users may be asked for an additional verification code during sign-in. These updates are designed to make accessing the app easier for users while also providing stronger protection for sensitive care information.

In addition to the larger redesign work, this update includes a wide range of quality-of-life improvements, fixes, and performance updates across the app. We addressed issues affecting screen behavior, layout consistency, and reliability in several areas, and continued polishing the overall experience on both phones and tablets. From onboarding and authentication to schedules, notes, notifications, and settings, this release brings a more stable and cohesive experience throughout WithinReach.

Overall, this update is focused on making WithinReach feel more modern, more secure, and more supportive in everyday use. With a redesigned Care Log, major improvements across every part of the Schedule experience, stronger sign-in and verification options, and a broad set of usability refinements, this release lays the foundation for a smoother and more dependable experience for patients, caregivers, and families alike.
