Hammond Home Health a secure iOS app that connects with Apple Health and allows users to share selected health and wellness data with the Hammond Home Health secure platform.

Patient Data can be accessed directly on the user’s device or through a secure database by authorized Hammond Home Health staff for monitoring, reporting, and for alert purposes. This enables improved patient visibility into health trends and supports care coordination with Hammond team for education and intervention to support healthy living at home. 
Supported data types may include activity, heart rate, sleep, blood glucose, respiratory data, and other Apple Health metrics, based on user authorization.
